After completing this electronic form, save and print a paper copy of the form. Date and sign it, and email it to askhr@hitachivantara.com and admin@jbenefits.com to complete your enrollment.

If you use a web-based email program such as Gmail or Yahoo in Windows 7, Windows Vista or Windows XP, you will need to save then attach your forms manually to your emails. Click here for more information.
